2022 Enrolment Form ​

This enrolment form has been designed to gather the necessary information about your child required to meet the legislative, funding, administrative, health, safety and operational considerations that allow us to provide your family with a quality early childhood education experience.

Please note the following:-

  1. We will require details of persons you authorise to collect your child and also persons you nominate as emergency contacts. We will ask for their Name, Phone Numbers and Driver's Licence Numbers. You will also need this information for all parents/caregivers, so please gather this information BEFORE you commence completing this online form. Please ensure you advise your authorised contacts that you have nominated them on this enrolment form.

  2. All questions marked with an asterisk * are compulsory.

    1. If you are a sole parent, please write N/A for the second parent sections.

    2. It is a requirement that at least one emergency contact person be provided. Forms will not be able to be submitted without providing at least one person. This can be a neighbour or a friend if you do not have any family living close by.

  3. If your child has specific medical conditions we will provide you with separate paperwork to be completed and returned to kindy.

  4. Questions relating to court orders and/or parenting orders are compulsory. Please write N/A if this does not apply to your family.

  5. Emergency medication - please note the questions related to permission for administration of asthma medication or adrenaline (epipen) are for the case of emergency only. The questions are NOT asking if your child has asthma or anaphylaxis, it is for the instance should an unexpected reaction occur. 

  6. Unfortunately there is no facility to save a partially completed form and return to it at a later time. Please allow approximately 20 to 30 minutes to complete and submit.

  7. Once you have submitted the form, you are unable to return to it. You can email mtgravattkindy@bigpond.com after submitting the form and we can email you a copy of your submitted enrolment form.
